Case Manager - care planning (San Jose, CA)

Cedar Maple Housing Group provides affordable homes and support services around San Jose. We are hiring a Case Manager to take on care planning. We value patience and follow-through. What you will do • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in San Jose. • Applications are reviewed on a rolling basis.
