Case Manager - care planning (San Jose, CA)

Stonegate Oak Care Services supports families and individuals in San Jose with personal care and coordination, and we need a Case Manager to lead our care planning. You would join a small organisation of about 130 people based in San Jose. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in San Jose, with some flexibility after the first few months. • We reply to every applicant.
