Case Manager - care planning (San Jose, CA)

Northwind Summit Property Management is looking for a Case Manager. The job centres on care planning for resident events, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in San Jose. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in San Jose. • Applications are reviewed on a rolling basis.
