Case Manager - care planning (San Diego, CA)

We are growing our team at Summit Cobalt Learning Group and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for learning materials. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with learning materials. • Bilingual English and Spanish.
