Case Manager - care planning (San Diego, CA)

Ironwood Ember Housing is a nonprofit housing provider in San Diego, and we need a Case Manager to lead our care planning. You would join a large organisation of about 366 people based in San Diego. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for repair requests. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in San Diego. • Applications are reviewed on a rolling basis.
