Case Manager - care planning (San Diego, CA)

Bluepeak Timber Property Management Group manages apartment communities around San Diego, and we need a Case Manager to lead our care planning. You would join a small organisation of about 192 people based in San Diego. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for unit turnovers. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Diego, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
