Case Manager - care planning (San Diego, CA)

Westbrook Birch Care Services Group provides in-home and community support in San Diego, and we need a Case Manager to lead our care planning. The work is personal and so is the culture. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in San Diego, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
