Case Manager - care planning (San Diego, CA)

Greenridge Vale Housing Group provides affordable homes and support services around San Diego, and we need a Case Manager to lead our care planning. We value patience and follow-through. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Full-time, Monday to Friday. • On site in San Diego, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
