Case Manager - care planning (San Diego, CA)

Rivermark Timber Housing is a nonprofit housing provider in San Diego, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in San Diego. • Applications are reviewed on a rolling basis.
