Case Manager - care planning (San Diego, CA)

Highbridge Loom Learning Group is looking for a Case Manager. The job centres on care planning for school calendars, and it would suit someone who likes steady, practical work. Families are partners. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in San Diego, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
