Case Manager - care planning (San Diego, CA)

We are growing our team at Bright Summit Property Management and adding a Case Manager whose main responsibility will be care planning. Residents are neighbours, not numbers. What you will do • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in San Diego. • We reply to every applicant.
