Case Manager - care planning (San Diego, CA)

Bright Basin Health is a healthcare provider in San Diego with clinics and support teams, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in San Diego. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in San Diego. • We reply to every applicant.
