Case Manager - care planning (San Antonio, TX)

Highbridge Nest Early Learning is an early-learning provider in San Antonio. We are hiring a Case Manager to take on care planning. Low staff turnover is a goal we take seriously. What you will do • Help with applications and appointments for meal programmes. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in San Antonio, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
