Case Manager - care planning (San Antonio, TX)

Clearpath Meadow Community Partners runs community programmes across San Antonio, and we need a Case Manager to lead our care planning. You would join a small organisation of about 47 people based in San Antonio. What you will do • Connect clients with partner agencies. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in San Antonio, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
