Case Manager - care planning (Salt Lake City, UT)

Bluepeak Thistle Early Learning is looking for a Case Manager. The job centres on care planning for meal programmes, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Full-time, on a regular day shift. • On site in Salt Lake City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
