Case Manager - care planning (Salt Lake City, UT)

Fernwood Loom Early Learning is an early-learning provider in Salt Lake City, and we need a Case Manager to lead our care planning. Children come first and so does looking after the teachers. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Salt Lake City. • We reply to every applicant.
