Case Manager - care planning (Salt Lake City, UT)

Highbridge Mill Care Services provides in-home and community support in Salt Lake City, and we need a Case Manager to lead our care planning. Teams support each other, especially on hard days. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Salt Lake City. • Applications are reviewed on a rolling basis.
