Case Manager - care planning (Sacramento, CA)

Cedar Keystone Health is looking for a Case Manager. The job centres on care planning for referrals, and it would suit someone who likes steady, practical work. You would join a small organisation of about 40 people based in Sacramento. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals.
