Case Manager - care planning (Sacramento, CA)

We are growing our team at Nova Summit Property Management and adding a Case Manager whose main responsibility will be care planning. We are flexible about schedules. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with maintenance requests. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Sacramento, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
