Case Manager - care planning (Richmond, VA)

Cedar Keystone Health provides primary and specialty care in Richmond, and we need a Case Manager to lead our care planning. You would join a small organisation of about 28 people based in Richmond. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Richmond. • We will tell you what to expect at each step of the hiring process.
