Case Manager - care planning (Richmond, VA)

Westbrook Yarrow Civic Services works with county and city offices around Richmond, and we need a Case Manager to lead our care planning. We follow procedures carefully and explain them clearly. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for budget submissions. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with budget submissions. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Richmond, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
