Case Manager - care planning (Richmond, VA)

Clearpath Kestrel Housing provides affordable homes and support services around Richmond. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 14 people based in Richmond. What you will do • Help with applications and appointments for resident services. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Full-time, with a steady weekday schedule. • On site in Richmond, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
