Case Manager - care planning (Richmond, VA)

We are growing our team at Greenridge Loom Early Learning and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Richmond. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Richmond. • We reply to every applicant.
