Case Manager - care planning (Reno, NV)

Stonegate Ashton Health runs clinics and patient services around Reno. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Reno, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
