Case Manager - care planning (Reno, NV)

Westbrook Rowan Health Group runs clinics and patient services around Reno. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Reno. • We will tell you what to expect at each step of the hiring process.
