Case Manager - care planning (remote)

We are growing our team at Foxglove Sable Community Partners and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Omaha. • We reply to every applicant.
