Case Manager - care planning (remote)

Highbridge Bright Community Partners Group is a Miami charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Miami. • We reply to every applicant.
