Case Manager - care planning (remote)

Lumen Quill Care Services is a social-care provider serving older adults and people with disabilities around Miami. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 761 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Miami. • Applications are reviewed on a rolling basis.
