Case Manager - care planning (remote)

Clearpath Union Care Services provides in-home and community support in Spokane, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Spokane. • Applications are reviewed on a rolling basis.
