Case Manager - care planning (remote)

We are growing our team at Bright Quarry Care Services and adding a Case Manager whose main responsibility will be care planning. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 16 hours a week, afternoons. • Fully remote, with quarterly visits in Boise. • We reply to every applicant.
