Case Manager - care planning (remote)

Cedar Drift Health is looking for a Case Manager. The job centres on care planning for clinic supplies, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish.
