Case Manager - care planning (remote)

We are growing our team at Bluepeak Rowan Health Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 126 people, working mostly from home. What you will do • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Fresno. • We reply to every applicant.
