Case Manager - care planning (remote)

Ironwood Glen Civic Services Group is looking for a Case Manager. The job centres on care planning for procurement, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for procurement. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with quarterly visits in Tulsa. • Applications are reviewed on a rolling basis.
