Case Manager - care planning (remote)

Clearpath Sable Community Partners is looking for a Case Manager. The job centres on care planning for benefits enrolment, and it would suit someone who likes steady, practical work. Flexibility and kindness are expected. What you will do • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with quarterly visits in Louisville. • We reply to every applicant.
