Case Manager - care planning (remote)

Highbridge Beacon Early Learning is an early-learning provider in Salt Lake City, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Salt Lake City. • We reply to every applicant.
