Case Manager - care planning (remote)

Bluepeak Pine Housing Group provides affordable homes and support services around San Jose, and we need a Case Manager to lead our care planning. Collaboration with local agencies is constant. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident services. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Fully remote, with an occasional day on site in San Jose. • Applications are reviewed on a rolling basis.
