Case Manager - care planning (remote)

We are growing our team at Silverline Iron Learning Group and adding a Case Manager whose main responsibility will be care planning. We follow the school calendar and we like people who like learning. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Tucson. • We will tell you what to expect at each step of the hiring process.
