Case Manager - care planning (remote)

Greenridge Pine Community Partners is looking for a Case Manager. The job centres on care planning for benefits enrolment, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Phoenix. • Applications are reviewed on a rolling basis.
