Case Manager - care planning (remote)

Foxglove Sable Community Partners is looking for a Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. Flexibility and kindness are expected. What you will do • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Houston. • We reply to every applicant.
