Case Manager - care planning (remote)

Silverline Pine Community Partners Group is a Charlotte charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. We are small enough to know everyone's name. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for benefits enrolment. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Charlotte. • Applications are reviewed on a rolling basis.
