Case Manager - care planning (remote)

We are growing our team at Clearpath Drift Health Group and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 976 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with quarterly visits in Oakland. • Applications are reviewed on a rolling basis.
