Case Manager - care planning (remote)

We are growing our team at Lumen Quill Health and adding a Case Manager whose main responsibility will be care planning. Continuing education is supported. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with quarterly visits in Savannah. • We reply to every applicant.
