Case Manager - care planning (remote)

We are growing our team at Clearpath Vista Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 829 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Reno. • We will tell you what to expect at each step of the hiring process.
