Case Manager - care planning (remote)

We are growing our team at Redstone Summit Learning Group and adding a Case Manager whose main responsibility will be care planning. Collaboration between teachers and staff is strong. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for school calendars. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Knoxville. • Applications are reviewed on a rolling basis.
