Case Manager - care planning (remote)

Westbrook Thistle Early Learning Group runs childcare centres in Hartford caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for meal programmes. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Hartford. • We reply to every applicant.
