Case Manager - care planning (remote)

We are growing our team at Rivermark Meadow Community Partners Group and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Hartford. • We reply to every applicant.
