Case Manager - care planning (remote)

Nova Yarrow Civic Services is looking for a Case Manager. The job centres on care planning for case files, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for case files. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Tampa. • We will tell you what to expect at each step of the hiring process.
