Case Manager - care planning (remote)

We are growing our team at Rivermark Junction Property Management and adding a Case Manager whose main responsibility will be care planning. Residents are neighbours, not numbers. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for unit turnovers. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Chicago. • We reply to every applicant.
