Case Manager - care planning (remote)

Cedar Pike Community Partners is looking for a Case Manager. The job centres on care planning for client intake, and it would suit someone who likes steady, practical work. You would join a small organisation of about 26 people, working mostly from home. What you will do • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Indianapolis. • We will tell you what to expect at each step of the hiring process.
