Case Manager - care planning (remote)

Stonegate Iron Early Learning runs childcare centres in Savannah caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our care planning. We celebrate small wins and steady routines. What you will do • Help with applications and appointments for tuition and subsidies. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with tuition and subsidies. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Savannah. • We will tell you what to expect at each step of the hiring process.
